Resolution fails on dual-head video cards. Tried Nvidia 68xx and 74xx and a ATI Rage. When the game loads and detects dual-head video, it offers one and only one resolution: full screen spanning across both monitors. It should offer single-screen full screen (one monitor). It is impossible to play the game spanning across two monitors.

Also of note: When in window mode, the window is set to full-screen size and the mouse is taken over so it is impossible to resize the window. The end result is that the game is unplayable in full-screen or windowed mode on a dual-head system.
